Job summary

Reserve Care (formerly Hospice of the Western Reserve) invites you to join our team as an On-Call Nurse. You will provide after-hours nursing care in patients' homes, delivering comfort and support to patients and families when they need it most.

We value compassion, excellence, and integrity. Ohio-licensed RNs with strong communication skills and the ability to work independently in dynamic home environments are encouraged to apply. Schedule from 8:00 am to 8:30 pm.

Qualifications

• High School Diploma or equivalent.
• Graduate of an accredited school of nursing; Bachelor's degree preferred.
• Licensed RN in the state of Ohio.
• Experience in pain/symptom management, oncology, or mental health preferred.
• Effective written and oral communication.
• Ability to work independently and with a team.
• Ability to care for all age groups with sound professional judgment.
• Ability to handle stress and adapt to changing conditions.
• Ability to visit patients in homes in various weather conditions.
• Proficiency in MS Office; Word/Excel; donor/grant portals and virtual meeting tech.
• Valid driver's license with good driving record.

Responsibilities

• Assess physical, psychosocial and spiritual needs of patient and family (RN only).
• Provide intermittent skilled nursing services under physician direction.
• Monitor and evaluate pain and symptoms; advise patient and family on management.
• Serve as liaison to physician; relay information on patient needs.
• Provide emotional support to patient and family.
• Refer patients/families to other services when appropriate.
• Provide health care teaching at the residence.
• Make visits to patient residence at the time of death.
• Maintain and complete documentation promptly.
• Communicate with team while respecting confidentiality and dignity.
• Relay pertinent information to appropriate team members.
• Participate in team meetings and mandatory trainings.
• RN triage coverage as needed.
• Perform other duties as assigned.
