Job summary

Traditions Health is seeking an On-call Hospice Nurse to respond to calls from patients, families and referral sources. You will assess needs, plan interventions, and coordinate with the interdisciplinary team in patient homes or care facilities.

The role requires RN licensure, BLS certification, and the ability to work independently during after-hours shifts. Experience in hospice or home health is preferred and travel is involved.

Qualifications

• Graduate of an accredited school of nursing.
• Valid RN license with current BLS certification.
• Experience in hospice or home health preferred; minimum 1 year.
• Must be able to work independently and manage after-hours calls.

Responsibilities

• Be available by phone/pager 5PM–9AM Mondays–Fridays and 24 hours on weekends.
• Identify and coordinate patient/family care for advanced illness.
• Document interventions and changes to care plans.
