Job summary

Reserve Care is seeking an On-Call Nurse to provide compassionate, skilled nursing care in patients' homes after hours. The role emphasizes autonomy, collaboration with physicians, and support for patients and families during challenging times.

The position requires an Ohio RN license and strong communication skills, with experience in pain management or oncology preferred. A schedule from 8:00 am to 8:30 pm supports flexible coverage in the community.

Qualifications

• RN licensed in the state of Ohio.
• Experience in pain and symptom management, oncology, or mental health preferred.
• Able to communicate effectively, orally and in writing.
• Ability to work independently and collaboratively with a team.

Responsibilities

• Assess physical, psychosocial, and spiritual needs of patients and families.
• Provide intermittent skilled nursing services under physician direction.
• Monitor and evaluate pain and symptoms; advise patients and families.
• Serve as liaison to physicians and provide information based on patient needs.
• Provide emotional support to patient and family.
• Refer patients/families to other services when appropriate.
• Provide health care teaching to patients and families at home.
• Perform visits to patient residences as needed, including end-of-life care.
• Maintain and complete documentation promptly and accurately.
• Communicate with patients, families, and health professionals while maintaining confidentiality.
