Job summary

Superior Hospice, part of the Home Health Care, Inc. family, seeks a Hospice On-Call Registered Nurse (RN) to handle after-hours calls in the Twin Cities Metro. Qualifications

• Active Minnesota RN license.
• Hospice experience preferred; ER/ICU/oncology/home health/palliative care nurses encouraged.
• Strong assessment, critical thinking and clinical decision-making skills.
• Confidence working independently in unpredictable situations.
• Compassionate communication with a calm, reassuring presence.

Responsibilities

• Respond to after-hours hospice calls across the Twin Cities Metro.
• Assess urgent changes in patient condition and provide symptom management.
• Complete emergency visits and implement comfort-focused interventions.
• Educate and reassure patients, families and caregivers.
• Coordinate care with physicians and interdisciplinary hospice team.
• Maintain accurate and timely EMR documentation.
