Superior Hospice is seeking an on-call Registered Nurse (RN) to respond to after-hours hospice calls, perform urgent nursing visits, and coordinate care with an interdisciplinary team in Minnesota. Qualifications

• Active Minnesota RN license.
• Hospice or end-of-life care experience preferred.
• Ability to work independently in urgent clinical situations.
• Strong assessment and communication skills.
• Evening and overnight on-call availability required.

Responsibilities

• Respond to after-hours hospice calls and urgent patient needs.
• Perform emergency nursing visits for symptom management, pain control, medication administration, and clinical assessment.
• Provide telephone support and education to patients, families, and caregivers.
• Collaborate with physicians and the interdisciplinary hospice team to coordinate care.
• Complete accurate and timely EMR documentation.
