Job summary

Superior Hospice, part of the Home Health Care, Inc. family, is seeking an On-Call Hospice RN in Lakeville, Minnesota. Requires an active Minnesota RN license; hospice, home health, oncology, ICU, or palliative care experience is encouraged.

Qualifications

• Active Minnesota RN license is required.
• Hospice or end-of-life care experience preferred.
• Ability to work independently across after-hours calls.
• Excellent assessment and critical thinking skills.
• Strong communication and compassionate patient care.

Responsibilities

• Respond to after-hours hospice calls and urgent patient needs.
• Perform emergency nursing visits for symptom management, pain control, medication administration, and clinical assessment.
• Provide telephone support and education to patients, families, and caregivers.
• Collaborate with physicians and the interdisciplinary hospice team to coordinate care.
• Complete accurate and timely EMR documentation.
