Job summary

Gentiva's Heartland Hospice is seeking a dedicated Registered Nurse (RN) for on-call hospice care in Wyomissing, PA. The RN will respond to after-hours patient and family needs, ensuring continuity of care during evenings, overnights, weekends, and holidays.

The role offers a unique schedule with a full week on duty followed by a full week off, emphasizing compassionate, patient-centered care. The ideal candidate will have an active RN license in Pennsylvania, CPR certification, and at least 1

Qualifications

• Current RN license in the state of employment.
• CPR certification is required.
• Minimum 1 year of nursing experience, preferably in hospice or home health.

Responsibilities

• Provide direct patient care and after-hours support on-call.
• Serve as primary liaison between patients, families, physicians, and the hospice team.
• Respond to urgent calls and manage symptoms and care plans after hours.
• Document visits and communications accurately in EMR systems.
• Collaborate with LPNs/LVNs, CNAs, and other care team members to ensure continuity of care.
