Job summary

The Highlands at Wyomissing is seeking a compassionate Registered Nurse (RN) for our After-Hours Hospice team. You will perform admissions, patient visits, symptom management, and care coordination on a predictable 7-on/7-off schedule to deliver exceptional hospice care.

Responsibilities include comprehensive patient assessments, care planning with the IDG, medication management with physicians, and on-call coverage while supervising ancillary staff and coordinating with families.

Qualifications

• PA Registered Nurse License required
• CPR Certification required
• Must be licensed driver with insured vehicle

Responsibilities

• Perform hospice admissions and patient visits
• Provide symptom management and nursing care using the nursing process
• Develop and revise care plans with IDG and physician
• Coordinate care with HHA and on-call staff
• Participate in on-call duties and ensure necessary equipment
• Attend continuing education and maintain competencies
