Job summary

AccentCare is seeking an After Hours Registered Nurse (RN) for hospice in King of Prussia, PA. The role supports care for patients across Bucks, Montgomery, Philadelphia, Delaware, and Chester Counties with a 5pm–8am, 7 on/7 off schedule.

The position is full-time, requiring active RN licensure and hospice/home-health/medical-surgical experience. Strong communication, flexibility for on-call duties, and proficiency with EMR systems are essential.

Qualifications

• Current licensure as a Registered Nurse in the applicable state is required.
• Associate degree in nursing preferred.
• CPR certification is recommended (required for direct patient care in Georgia).
• Experience in hospice, home health, medical-surgical, or geriatrics/pediatrics is preferred.

Responsibilities

• Conducts comprehensive nursing assessments and collaborates with patients, families, physicians, and IDG to develop, implement, and evaluate care plans.
• Maintains timely documentation and ensures plans reflect current patient status.
• Provides education and emotional support to patients and caregivers and participates in on-call rotations, admissions, and transitions.
• Coordinates all aspects of patient care including scheduling, medication management, plan of care oversight, and supervision of LPNs/LVNs and Hospice Aides when acting as Care Manager.
• Responds to triage calls, performs visits and assessments, and participates in IDG meetings as assigned.
