Job summary

Continuum Hospice is seeking a Full Time On-Call Registered Nurse (RN) to join our team in Santa Cruz and surrounding areas, providing compassionate hospice care during critical moments and coordinating patient needs.

This on-call role requires a current RN license, CPR certification, EMR experience, and a willingness to work a 7 on/7 off schedule. Competitive compensation and a supportive benefits package are included.

Qualifications

Hold current unencumbered license as a Registered Nurse.
One (1) year experience as a professional nurse within the last three (3) years.
Hospice/home health experience preferred but not required.
Must be computer proficient in typing and various programs, including a background in EMR.

Provide Empathetic and Heartfelt Assessments: Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
Maintain Accurate Documentation: Every detail is crucial in delivering high-quality care, both in the patient's home and within our agency and regulatory standards.
Provide Support for Families: Impact patients' families by sharing your knowledge and expertise, guiding and supporting them in caring for their terminally ill loved ones.
