Job summary

Continuum Hospice seeks a Full Time On-Call Registered Nurse to provide compassionate hospice care in the Santa Cruz area. The role involves responding to patient needs, coordinating care, and supporting families during critical moments.

Requirements include an active RN license, a nursing degree or recent nursing experience, CPR certification, and EMR proficiency. Hospice/home health experience is preferred.

Qualifications

• Current unencumbered RN license.
• Bachelor's degree in nursing or current nursing experience within last 3 years.
• Hospice/home health experience preferred but not required.

Responsibilities

• Be the Lifeline: respond to a variety of patient needs, from urgent to routine.
• Provide empathetic assessments of physical, emotional, social and spiritual needs.
• Maintain accurate documentation in patient home records and agency systems.
• Provide support for families, guiding and reassuring during end-of-life care.
