Job summary

Continuum Hospice & Palliative Care California seeks a Full Time On-Call Registered Nurse (RN) to join our team, delivering compassionate hospice care for patients at home and supporting families during critical moments. The RN coordinates care, assesses needs, and ensures timely interventions.

This on-call role covers Santa Cruz area, with a 7 days on/7 days off schedule; requires active RN license, NLN-accredited degree or equivalent, EMR proficiency, and CPR.

Qualifications

• Hold current unencumbered RN license.
• Bachelor's degree in nursing from NLN-accredited program, OR at least 1 year of experience as a professional nurse within the last 3 years.
• Hospice/home health experience preferred but not required.

Responsibilities

• Be the Lifeline: Respond to a variety of patient needs, from urgent to routine.
• Provide empathetic and heartful assessments of physical, emotional, social, and spiritual needs.
• Maintain accurate documentation in home and agency records.
• Provide support for families by guiding and supporting them in caring for terminally ill loved ones.

On Call for 7 Days on and 7 Days off from Friday at 5 pm - Monday 8 am, Monday 5 pm - Friday 8 am

Qualifications

• Hold current unencumbered license as a Registered Nurse.


• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR




• One (1) year experience as a professional nurse within the last three (3) years.




• Hospice/home health experience preferred but not required.


• Must be computer proficient in typing and various programs, including a background in EMR.


• Possess and maintains current CPR certification.

Pay

Approx. $135k annually with pay differentials paid hourly; may vary with hours worked. The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
