Job summary

Continuum Hospice is seeking a Full Time On-Call Registered Nurse (RN) to join our team in Santa Cruz and surrounding areas, providing compassionate hospice care during critical moments and coordinating patient needs.

This on-call role requires a current RN license, CPR certification, EMR experience, and a willingness to work a 7 on/7 off schedule. Competitive compensation and a supportive benefits package are included.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including a background in EMR.

As an On-Call RN, you will play a vital role in identifying patient and family needs, coordinating care, and offering preventative measures and compassionate support.

Schedule:

On Call for 7 Days on and 7 Days off from Friday at 5 pm - Monday 8 am, Monday 5 pm - Friday 8 am

Territory:

Santa Cruz and Surrounding Areas.

Qualifications

• Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR
• One (1) year experience as a professional nurse within the last three (3) years.
• Hospice/home health experience preferred but not required.
• Must be computer proficient in typing and various programs, including a background in EMR.
• Possess and maintains current CPR certification.

Pay:

Approx. $135k annually with pay differentials paid hourly; may vary with hours worked. The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
