Job summary

Ohio Living seeks an On Call/After Hours LPN or RN to respond to clinical and non-clinical situations after normal business hours in hospice and home health settings in Ohio. Qualifications

• Current unencumbered license to practice as LPN or RN in Ohio.
• Three years of acute care/geriatric nursing experience required.
• Proficiency with Windows, Microsoft Office and EMR preferred.

Responsibilities

• Triages calls to hospice and home health, identifies patient needs and reviews care plan.
• Conducts home/facility visits to observe changes and updates the plan of care.
• Communicates with care team and families about patient needs and changes in condition.
• Provides medication administration within LPN scope and ensures timely delivery.
• Documents patient care and maintains accurate records per standards.
