Ohio Living Village Home Health & Hospice is seeking an On-Call Registered Nurse to respond to clinical and non-clinical situations after hours. You will assess patients, triage calls, and provide skilled nursing care to hospice and home health patients and families to meet the plan of care.

This role emphasizes timely communication, adherence to standards, and collaboration with the Hospice and Home Health teams. Evening, night, weekend, and holiday shifts are required.

Qualifications

• Current unencumbered Ohio RN license.
• Three years experience in acute care, medical-surgical, oncology, or geriatric nursing.
• Proficiency with Windows, Microsoft Office, and internet.
• Experience in a Medicare-certified home health agency preferred.

Responsabilités

• Triage calls from patients and caregivers and assess needs.
• Develop and implement a care plan aligned with the plan of care.
• Conduct home visits and adjust the plan as needed.
• Educate patients and families on pain, symptom management and care goals.
• Ensure timely administration of medications and documentation.
