Job summary

Ohio Living is seeking an On-Call/After Hours Registered Nurse to respond to clinical and non-clinical situations after normal business hours for hospice and home health patients. You will assess patients, implement care plans, administer medications, and coordinate with interdisciplinary teams to ensure quality, compliant care.

The role requires an Ohio RN license (BSN preferred) and experience with EMR systems.

Qualifications

• Current unencumbered RN license in Ohio.
• BSN preferred.
• Proficiency with Windows, Word, Excel, PowerPoint and internet.
• EMR experience preferred.

Responsibilities

• Respond to clinical and non-clinical situations after normal business hours (evenings, nights, weekends, holidays).
• Provide skilled nursing assessment, interventions and supportive care per plan of care.
• Complete hospice/home health admissions assessments and initial care plans.
• Communicate with hospice and home health teams about changes in condition and patient needs.
• Deliver medications and manage orders for timely patient care.
• Maintain accurate patient records per regulatory standards.
• Participate in Hospice Interdisciplinary Group meetings as needed.
• Maintain high-level customer service for internal and external customers.
