Job summary

Mosaic is seeking a Mosaic at Home Coordinator/Case Manager to empower individuals with intellectual and developmental disabilities. You will build relationships, coordinate Individual Support Plans, monitor service delivery, and ensure compliance as part of a mission-driven team.

The role involves on-call support, field visits within the Des Moines area, and travel up to two hours. A Bachelor’s in human services and 2+ years of related experience are preferred.

Qualifications

• Bachelor’s degree in human services or related field (preferred).
• Minimum of two years related work experience.
• Previous work with people with intellectual disabilities is a plus.
• Valid driver's license in good standing for home visits.
• Strong communication, relationship-building, problem-solving abilities, and computer proficiency.
• Experience with HCBS industry regulations.

Responsibilities

• Build connections with individuals served, families, host home providers, and direct care workers.
• Help individuals express goals and ensure their voices are heard and respected.
• Monitor relationships between individuals and home providers to ensure a positive match.
• Collaborate with the service planning team to create Individual Support Plans.
• Oversee service delivery and contract compliance through home visits.
• Provide on-call support to providers and direct care workers.
