Job summary

Mosaic seeks a dedicated Coordinator/Case Manager to empower individuals in the Des Moines area. You will build strong relationships with clients, families, and care providers, advocate for voices to be heard, and ensure personalized support through planned services and regular field visits.

As a central bridge, you’ll monitor plan delivery, ensure regulatory alignment, and provide on-call support to staff, while traveling up to two hours from Des Moines as needed.

Qualifications

• Bachelor’s degree in human services or related field and at least two years of related work experience.
• Experience working with people with intellectual disabilities is a huge plus.
• A valid driver's license in good standing for home visits.
• Strong communication, relationship-building, problem-solving, and computer skills.
• Experience in behavior management and HCBS industry regulations.

Responsibilities

• Build epic connections with individuals, families, host homes, and direct care staff to ensure excellent service.
• Help individuals express goals and ensure their voices are heard and respected.
• Monitor relationships between individuals and home providers to ensure a good match.
• Collaborate to create Individual Support Plans aligning wishes with policies and regulations.
• Oversee service delivery and conduct regular home and environment visits for contract compliance.
• Provide on-call support to providers and direct care workers when needed.
