Job summary

Mosaic is seeking a dedicated Mosaic at Home Coordinator/Case Manager in Central Iowa-Des Moines Metro area. You will bridge individuals with families and providers, align plans with HCBS rules, and travel for home visits with an agency vehicle or mileage reimbursement.

You’ll build strong relationships, champion client goals, monitor service delivery, and provide on-call support to the care team in a collaborative, mission-driven environment.

Qualifications

• Bachelor's degree in human services or related field preferred.
• Minimum of two years of related work experience.
• Valid driver's license in good standing; able to travel for home visits.
• Excellent communication and relationship-building skills.
• Experience with HCBS regulations and person-centered planning.

Responsibilities

• Build epic connections with individuals, families, host home providers, and direct care workers; deliver exceptional customer service.
• Help individuals express goals, dreams, and needs; ensure their voices are heard and respected.
• Monitor relationships between individuals and home providers to ensure a positive match.
• Collaborate with the service planning team to create and weave Individual Support Plans.
• Oversee service delivery and contract compliance through home and service environment visits.
• Provide on-call support to providers and direct care workers when issues arise.
