Job summary

Mosaic invites you to join as a Mosaic at Home Coordinator/Case Manager, a salaried role with on-call support and travel for home visits up to two hours from Des Moines. You’ll build meaningful relationships with individuals you serve, their families, host homes, and direct care workers.

As part of the team, you’ll develop Individual Support Plans, ensure service delivery compliance, and provide on-call support.

Qualifications

• Bachelor’s degree in human services or related field with minimum two years of related experience.
• A valid driver’s license for home visits.
• Top-notch communication, relationship-building, problem-solving, and computer skills.

Responsibilities

• Build strong relationships with individuals, families, host homes and direct care workers.
• Help individuals express goals and ensure their voices are respected.
• Monitor matches between individuals and home providers for compatibility.
• Collaborate to create Individual Support Plans aligned with wishes and regulations.
• Oversee service delivery and compliance through home visits.
• Provide on-call support to providers and direct care workers.
