Job summary

Mosaic is seeking a dedicated Mosaic at Home Coordinator/Case Manager in Central Iowa to empower individuals with disabilities to live meaningful lives. You will build strong relationships, develop individualized plans, and ensure high-quality service delivery in home settings.

You’ll travel to provide visits, support families, and work closely with care teams to advocate for person-centered goals and regulatory compliance. This role blends field work with collaborative planning and reporting.

Qualifications

• Bachelor's degree in human services or related field required.
• Two+ years of related work experience preferred (IDD experience a plus).
• Valid driver's license in good standing.

Responsibilities

• Build epic connections with individuals, families, host homes and care workers.
• Be a dream champion by helping individuals express goals and needs.
• Monitor relationships to ensure positive matches with home providers.
• Architect personalized plans with service planning team and regulatory compliance.
• Oversee service delivery and conduct home/service environment visits.
• Provide on-call support to providers and direct care workers.
