Job summary

Mosaic seeks a Mosaic at Home Coordinator/Case Manager to empower individuals in Central Iowa with personalized supports and meaningful daily lives. You will build relationships with individuals, families, host homes, and direct care staff, advocating for their goals and ensuring strong communication.

You will conduct home visits, help design individualized plans, monitor service delivery, and provide on-call support to providers.

Qualifications

Bachelor’s degree in human services or related field; two years of related work experience preferred.
Previous experience working with people with intellectual disabilities is a plus.
Valid driver’s license in good standing (home visits).
Strong communication, relationship-building, problem-solving abilities and computer proficiency.
Knowledge of HCBS industry regulations is advantageous.

Responsibilities

Build rock-solid connections with individuals, families, host home providers, and direct care workers.
Champion individuals' goals and ensure their voices are heard and respected.
Monitor relationships between individuals and home providers to ensure positive matches.
Collaborate with service planning to create individualized support plans.
Oversee service delivery and contract compliance by conducting home visits.
Provide on-call support to providers and direct care workers as needed.
