Job summary

Mosaic is seeking a Home Coordinator/Case Manager to empower people with intellectual and developmental disabilities in the Des Moines metro area. You will build relationships with individuals, families, host home providers, and direct care workers to deliver personalized support.

You will oversee service delivery, collaborate on Individual Support Plans, and ensure policy and regulatory compliance during home visits and on-call shifts.

Qualifications

Bachelor's degree in human services or related field preferred.
Minimum two years of related work experience required.
Valid driver's license in good standing is required.

Responsibilities

Build relationships with individuals we support, their families, host home providers, and direct care workers.
Help individuals express goals and ensure their voices are respected.
Monitor relationships between individuals and home providers to ensure positive matches.
Collaborate with the service planning team to create Individual Support Plans.
Oversee service delivery and contract compliance during home visits and while in the field.
Provide on-call support to providers and direct care workers.

Education

Bachelor's degree in human services or related field
