Job summary

Mercer County Area Agency on Aging is seeking a dedicated professional to support the Long-Term Care Ombudsman Program and PA MEDI services in Mercer County. The role involves visiting facilities, counseling residents and families, and coordinating with community partners to protect rights and improve quality of life for older adults.

Qualifications

• Associate degree in Human Development & Family Studies or related field, or equivalent experience in health or social services.
• Experience working with older adults in a counseling, advocacy, or case management role is preferred.

Responsibilities

• Visit nursing facilities and personal care homes in Mercer County, providing training and technical assistance.
• Respond to and investigate consumer concerns and complaints in long-term care facilities.
• Foster independence and dignity by coordinating with families and resident councils and promoting positive relationships.
• Provide PA MEDI Medicare counseling, outreach, and education to older adults and caregivers.
