Job summary

The YMCA of Greater New York is seeking a dedicated part-time professional to support Older Adults through case management and resources. This role includes assessing individual needs, developing care plans, and maintaining client follow-up while ensuring confidentiality.

A Bachelor's degree in a related field and experience with Older Adults are necessary. The YMCA provides various benefits, including retirement options and a free membership, contingent upon employment status.

Qualifications

• Minimum of one year of experience in case management with Older Adults.
• Current CPR, AED, and First Aid certifications are required.
• Knowledge of local and citywide community services.

Responsibilities

• Conduct case management services for Older Adults and assist in accessing community resources.
• Develop an Individual Care Plan for each client seen in the program.
• Attend required staff meetings and participate in monthly event planning.

Skills

Experience with Older Adults

Bilingual Spanish speaker

Strong verbal and written communication skills

Education

Bachelor’s Degree in education, social work, mental health, or relevant field

Master’s degree preferred
