Job summary

Goddard Riverside in New York, NY is seeking a Case Manager to support seniors in living independently. You will help with applications for Medicare, Medicaid, SNAP, HEAP, and connect clients to medical, in-home, and legal services, with home visits and phone reassurance as needed.

Responsibilities include case management, referrals, and advocacy, developing care plans, and coordinating with aging services partners to ensure holistic support.

Qualifications

• Bachelor's degree in Social Work or Human Services, or 2 years of case management experience.
• Bilingual English/Spanish preferred and ability to handle confidential information.
• Strong communication, interpersonal, and written skills; flexible hours when needed.

Responsibilities

• Participate in staff meetings and interdisciplinary case conferences; assist with outreach.
• Ensure timely referrals and access to social services, wellness activities, and programs.
• Develop individualized service plans and reassess regularly with clients.
• Prepare case notes, assessments, and service logs per agency standards.
