Job summary

Neighborhood Health Center is seeking an Outreach & Enrollment Coordinator to support patient engagement and access to care across primary and dental services. You will identify OHP-eligible patients, assist with applications, coordinate care, and help patients overcome barriers to their first appointment.

The role includes data tracking, training new staff, and travel to clinic sites and events. Bilingual Spanish/English preferred, with HIPAA-compliant data handling.

Qualifications

• High school diploma or equivalent required; higher education preferred.
• One year of customer service or clerical experience required.
• Must be able to obtain OHP assister number.
• One year of related experience and/or training, or equivalent combination.
• Bilingual/bicultural proficiency (Spanish/English) strongly preferred.
• CHW training or experience preferred; Oregon Traditional Health Worker Registry certification not required.

Responsibilities

• Identify patients eligible for Oregon Health Plan and help with applications.
• Use roster tools to engage newly assigned NHC patients for primary and dental care.
• Reach out to new patients hospitalized or seen in ED to establish care.
• Assist dental patients in closing gaps in their care.
• Track outreach data and document in EHR and Excel.
• Provide in-person or virtual assistance for OHP applications/renewals.
• Ensure accurate attribution of patients to NHC with insurers.
• Help patients access in-network providers.
• Assist scheduling medical/dental appointments for lacking providers.
• Help new patients with MyChart and transportation arrangements.
• Train new employees in outreach/enrollment workflows.
• Participate in process/system improvements and care team meetings.
• Travel to clinic sites and community events as needed.
• Perform other duties as assigned.
