Job summary

Neighborhood Health Center in the Portland area seeks an Outreach & Enrollment Coordinator to support patient engagement and access to care. You will identify OHP eligibility, assist with applications, and coordinate care with primary and dental services.

The role requires strong communication, data tracking in EHRs, and ability to travel to clinic sites and events. Bilingual Spanish/English is strongly preferred and CHW training is a plus.

Qualifications

  • High school diploma or equivalent required; CHW training preferred.
  • 1 year of customer service or clerical experience required.
  • Must be able to obtain OHP assister number.
  • Bilingual Spanish/English preferred but not required.
  • Experience with EHR systems (OCHIN EPIC) and patient management tools preferred.

Responsibilities

  • Identify patients eligible for Oregon Health Plan and help with applications.
  • Reach out to new patients to facilitate engagement in primary and dental care.

SUMMARY

The Outreach & Enrollment Coordinator plays a vital role in supporting patient engagement and access to care at Neighborhood Health Center (NHC). This position focuses on outreach, patient assistance and care coordination to ensure that individuals are connected to primary care, dental services and health coverage. The Outreach & Enrollment Coordinator identifies and assists patients with Oregon Health Plan enrollment and renewal, facilitates new patient access to care including overcoming barriers that prevent them from attending their first appointment and supports efforts to maintain accurate provider attribution.

Essential Job Duties
  • Identify patients eligible for the Oregon Health Plan and conduct outreach to offer assistance with applications.
  • Utilize roster management tools to identify and reach out to newly assigned NHC patients to facilitate engagement in primary and dental care.
  • Identify and contact new patients assigned to NHC who have been hospitalized or visited the emergency department offering an appointment to establish care.
  • Perform outreach to assist dental patients assigned to NHC in closing gaps in their care.
  • Track outreach data and document interactions in the electronic health record (EHR) and excel spreadsheet.
  • Provide in-person or virtual assistance for patients and community members in applying for or renewing OHP coverage, including navigating application updates and understanding benefits.
  • Ensure accurate attribution of patients to NHC with their insurance payors.
  • Support patients with out-of-network insurance in accessing in-network providers.
  • Support patients in scheduling medical or dental appointments if lacking a primary care or dental provider.
  • Assist new patients in reducing barriers to care such as setting up MyChart accounts or connecting to transportation services for their first appointments.
  • Assist with training new employees in outreach and enrollment workflows and strategies.
  • Participate in process and system improvement initiative to enhance department services.
  • Attend care team meetings as appropriate.
  • Travel to clinic sites and community events as needed.
  • Perform other duties as assigned.
Qualifications: Education and/or Experience
  • High school diploma, or equivalent, is required; some higher-level education or vocational training is preferred.
  • One year of customer service-related experience, or clerical related experience, is required.
  • Must be able to obtain OHP assister number.
  • One year of related experience and/or training, or equivalent combination of education and experience, is required.
  • Bilingual/bicultural proficiency (Spanish/English spoken and written) is strongly preferred, but not required.
  • Community Health Worker (CHW) training or experience is preferred, certification on the Oregon Traditional Health Worker Registry is not required.
Knowledge, Skills, Abilities & Behaviors
  • Understanding of OHP eligibility, enrollment, renewal, and benefits.
  • Familiarity with health insurance programs related to OHP.
  • Knowledge of using EHR systems (OCHIN EPIC).
  • Understanding of patient engagement and care coordination.

  • Strong interpersonal skills to assist patients with diverse cultural and socio-economic backgrounds
  • Ability to identify and address patient care challenges (e.g., scheduling, MyChart, transportation).

WORKING CONDITIONS
  • Regularly sit while working on the computer; use hands and fingers to handle, control or feel objects tools or controls; repeat the same movements when entering data; ability to differentiate between colors, shades and brightness; read from a computer screen for extended periods of time.
  • Frequently stand and walk around the office to gather supplies, required to do filing, use office equipment or collaborate with employees.
  • Occasionally stand, stoop and lift or move objects, equipment and supplies weighing approximately 10-25 pounds.
  • The noise level in the work environment is usually moderate.
  • This position will require occasional travel.
