Job summary

Universal Community Health Clinic is seeking an Enrollment and Outreach Coordinator to oversee enrollment and in-reach activities ensuring timely access to care for all patients.

You will assist patients with Medi-Cal and private plans, screen eligibility, conduct renewals, educate on financial assistance and sliding scales, and collaborate with clinicians to expand coverage.

A HIPAA-compliant approach and accurate EHR documentation are required in this role.

Qualifications

• Assist patients with completing applications for Medi-Cal, private insurance, and other programs.
• Screen patients for insurance eligibility and provide program guidance.
• Conduct renewals for Medi-Cal and other programs.
• Maintain accurate documentation in EHR systems.
• Demonstrate HIPAA compliance in all duties.

Responsibilities

• Oversee enrollment and in-reach activities to ensure timely access to care.
• Review enrollment progress and assign staff to outreach tasks.
• Educate patients on financial assistance and sliding scale options.
• Participate in community outreach and enrollment events as needed.
• Collaborate with clinicians and staff to identify qualified patients for coverage.
