Qualifications

• Bilingual Spanish (spoken and written) required.
• 2+ years in outreach, care coordination, call center, enrollment, or related roles.

Responsibilities

• Conduct ongoing outreach to engage members within SLA.
• Deliver script-based outreach calls with tailored messaging.
• Screen for eligibility and guide enrollment steps.
• Assess interest and drive enrollment; provide resources if not eligible.
• Respond to member inquiries with clear, compassionate communication.
• Support program activation and schedule next-step appointments.
• Document outreach activity and enrollment status in the EHR.
