Job summary

Continuum Therapy Partners, LLC seeks a Certified Occupational Therapy Assistant to provide direct patient care at our Sturgeon Bay, WI location. The role includes implementing treatment plans, documenting progress, and educating patients on adaptive equipment and home exercise programs under OT supervision.

The position is PRN with daytime hours, supporting a patient-focused team in a multi-site network across several states. Wisconsin licensure and NBCOT certification are required.

Qualifications

• Associate Degree in Occupational Therapy Assistant Program required.
• Current, unencumbered state COTA License; NBCOT Certification required.
• 1 year of previous experience as a Certified Occupational Therapy Assistant preferred.
• Implements individualized treatment plans under the supervision of an Occupational Therapist.
• Documents patient progress and reports changes in patient status to the supervising OT.
• Educates patients on adaptive equipment and home exercise programs.
