Job summary

Innovate Rehab and Wellness in Missouri is seeking a Licensed Certified Occupational Therapy Assistant (COTA) to support high-quality OT services and help residents regain independence in a skilled nursing setting.

You will work under OT supervision, collaborate with nursing, PT, SLP, and other staff, document in the EMR, and participate in discharge planning with a patient-centered approach. Competitive hourly pay based on experience.

Qualifications

• Associate’s degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state OTA license.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Able to work independently and as part of a team.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
