Innovate Rehab and Wellness in Cassville, MO is seeking a Licensed Occupational Therapy Assistant (COTA) for PRN shifts in a Skilled Nursing Facility. You will deliver high-quality OT services, help residents regain independence, and provide therapy under OT supervision.

The role emphasizes one-on-one patient care, collaboration with the care team, and continuous documentation to support positive clinical outcomes and patient satisfaction.

Formación

• Associate degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.

Responsabilidades

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
