Job summary

Innovate Rehab and Wellness is seeking a Licensed Occupational Therapist for a full-time role in a skilled nursing facility. The OT will evaluate patients, design individualized plans, and help regain independence. You’ll collaborate with the care team and educate patients and caregivers on exercises and devices.

Applicants should hold a master’s or doctorate in OT with an active state license. Strong communication, problem-solving, and CPR certification (preferred) are valued in this

• Master’s or Doctorate in Occupational Therapy (accredited program).
• Valid state OT license.
• Experience as an OT preferred; new grads welcome.
• Strong communication and problem-solving skills.
• CPR certification preferred.

Responsibilities

• Evaluate patients’ functional abilities and therapy needs.
• Create and deliver individualized treatment plans.
• Help patients improve motor skills, coordination, and daily function.
• Support patients in regaining independence and quality of life.
• Conduct thorough OT evaluations and adjust plans as needed.
• Train patients and caregivers on exercises and devices.
• Educate on strategies to improve function and safety.
• Collaborate with PT, SLP, physicians and the care team.
• Document progress and treatment changes accurately.
• Stay current with OT best practices and research.
