Job summary

Evolve Therapy Services is seeking an Occupational Therapist for an in-house Skilled Nursing Facility in North Platte, NE. You will plan, organize, develop, and direct OT services to ensure high-quality resident care in compliance with standards.

Responsibilities include screening, evaluation, treatment planning, scheduling, documentation, and supervision of COTA, with opportunities for growth within the organization.

Qualifications

• Degree in Occupational Therapy.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention.
• Evaluate residents to develop treatment plans and goals.
• Communicate evaluation findings and plans to residents, families/caregivers, and interdisciplinary team members.
• Establish individual resident therapy schedules with the team.
• Deliver therapy per treatment plan and document/bill services.
• Perform all other duties as assigned to effectively treat patients.
• Oversee COTA and ensure documentation and treatments meet standards and reimbursement practices.

Education

OT degree

Active OT license in practicing state(s)

Continuing education to maintain licensure
