Evolve Therapy Services is hiring an Occupational Therapist for an in-house Skilled Nursing Facility located in North Platte, NE. The role includes direct therapy, evaluation, planning, and documentation to ensure high-quality resident care.

Qualifications

• Degree in Occupational Therapy is required.
• Must have an Active Occupational Therapy license in practicing state.
• Ongoing continuing education to maintain licensure.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents to inform treatment plans and set goals.
• Communicate findings and plans to residents, families, and team.
• Schedule resident therapy in coordination with team.
• Provide therapy and document/bill services according to plan.
• Perform additional duties to ensure patient care.
• Oversee COTA and ensure documentation and reimbursement practices are followed.
