Job summary

Evolve Therapy Services, LLC is seeking a licensed Occupational Therapist for an in-house Skilled Nursing Facility in North Platte, Nebraska. The role focuses on planning, directing OT services, and ensuring high-quality resident care.

You will screen residents, develop treatment plans, supervise COTA as needed, and collaborate with an interdisciplinary team. This is a full-time position with a relocation bonus option and flexible scheduling.

Qualifications

• Degree in Occupational Therapy required.
• Active Occupational Therapy license in practicing state(s).
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Conduct regular resident screenings to determine best intervention or treatment.
• Evaluate residents and develop individualized treatment plans with short- and long-term goals.
• Interpret and communicate evaluation findings and recommendations to residents, families, and the inter-disciplinary team.
