Job summary

Evolve Therapy Services, LLC is hiring an Occupational Therapist for an in-house Skilled Nursing Facility in North Platte, NE. This full-time role offers a $10,000 relocation or sign-on bonus and flexible scheduling with salaried pay options.

Part-time or PRN are also available, creating an adaptable work arrangement. The OT will plan, organize, develop, and direct therapy services, ensuring quality resident care while complying with federal, state, and local standards.

Qualifications

• Degree in Occupational Therapy is required.
• Active Occupational Therapy license in practicing state(s) is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine best intervention or treatment.
• Evaluate residents and develop treatment plans with goals.
• Communicate findings and plans to residents, families, and team.
• Schedule resident therapy with other team members.
• Provide therapy according to treatment plans and document/bill services.
• Oversee COTA and ensure documentation and treatments meet standards.
