Job summary

Evolve Therapy Services, LLC is seeking a compassionate Occupational Therapist for an in-house Skilled Nursing Facility in North Platte, NE. The role focuses on planning, directing OT services and collaborating with an interdisciplinary team to ensure high-quality resident care.

Responsibilities include screening and evaluating residents, developing treatment plans with goals, scheduling therapy, documenting and billing, and supervising a COTA as needed.

Qualifications

• Must have an active Occupational Therapy license in practicing state(s).
• Degree in Occupational Therapy is required.
• Ongoing continuing education to maintain licensure and certification.

Responsibilities

• Screen residents to determine the best intervention for each patient.
• Evaluate residents and develop treatment plans with short- and long-term goals.
• Interpret and communicate findings to residents, families, and the interdisciplinary team.
• Establish individual therapy schedules with the team.
• Deliver therapy per plan and ensure proper documentation and billing.
• Oversee COTA as needed and ensure documentation and reimbursement compliance.
• Perform additional duties as required to treat patients.
