Job summary

Sweetwater Care in Topeka, KS is seeking a compassionate Occupational Therapist to join our skilled nursing rehabilitation team. You will provide cognitive and ADL-focused therapy to residents, helping them regain independence and improve quality of life.

Qualifications

• Bachelor's degree or higher in Occupational Therapy or related field.
• Active Kansas state license.
• 1+ years of experience in a skilled nursing facility environment.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop a care plan.
• Use standardized assessments to support interventions.
• Provide rehabilitative, skilled treatment per regulatory requirements.
• Administer ADL retraining, therapeutic activities, exercises, modalities and assistive devices.
• Record progress and adjust treatments in the clinical record.
• Evaluate treatment outcomes and refer as needed.
• Discharge summary of services per regulatory requirements.
• Document all required information in the electronic health records.
• Supervise PT assistants, rehabilitation aides, or students as needed.
• Adhere to NBCOT/AOTA and state practice acts and company policies.
