Job summary

Sweetwater Care is seeking an Occupational Therapist to provide rehabilitative therapy in a skilled nursing facility, evaluating patients and developing care plans. You will document progress and collaborate with a multidisciplinary team to improve cognitive and ADL functioning.

Qualifications

• Bachelor's degree or higher in Occupational Therapy or related field.
• Active Ohio state license.
• 1+ years of experience in a skilled nursing facility preferred.

Responsibilities

• Screen/evaluate patients with cognitive and/or ADL disorders and develop a plan of care.
• Use standardized assessments to support interventions.
• Provide rehabilitative, skilled treatment per requirements.
• Supervise therapy assistants and aides as needed.
• Document minutes and outcomes in electronic records.
