Qualifications

• Active state license required.
• 2+ years of experience in a skilled nursing facility.

Responsibilities

• Screen and evaluate patients with cognitive/ADL disorders and develop care plans.
• Provide rehabilitative therapy per regulatory standards.
• Document treatment plans and progress in the EHR.
• Supervise assistants and aides as needed.
• Communicate patient progress to the health team and families.
