Job summary

Sweetwater Care is seeking a compassionate Occupational Therapist to join our skilled nursing team. The OT will provide rehabilitative therapy for residents, focusing on cognitive and ADL improvements and returning patients to prior functioning where possible.

The role reports to Rehabilitation Services and requires an active state license, a Bachelor's degree in Occupational Therapy, and at least one year of SNF experience. A true passion for elder care is essential.

Qualifications

• Bachelor's degree or higher in Occupational Therapy.
• Active Kansas state license.
• 1+ years of experience in a skilled nursing facility.

Responsibilities

• Screen and evaluate patients with cognitive/ADL disorders and plan care.
• Provide rehabilitative therapy per regulatory standards.
• Document treatment plans, progress, and outcomes in medical records.
• Supervise PTAs and rehabilitation aides when needed.
• Coordinate discharge summaries and communicate with care team.
