Responsibilities

• Screens/evaluates patients with cognitive and/or ADL disorders and develops care plans.
• Provides rehabilitative, skilled treatment per regulatory and clinical requirements.
• Documents services, minutes, and progress in the patient record.
• Supervises aides and participates in meetings and training.
• Communicates progress and plans with the health team and families.
