Continuum Therapy Partners, LLC seeks an Occupational Therapist to join our PRN team in Lake City, FL. You’ll enjoy flexible scheduling and be part of a team led by experienced clinicians who prioritize quality patient care and collaboration.

The role focuses on delivering individualized therapy, documenting patient progress, and educating families on equipment and home safety to empower independence and safe daily living.

Competenze

• Master's or Doctorate Degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Mansioni

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
