Job summary

Continuum Therapy Partners seeks an Occupational Therapist for Windsor Health & Rehabilitation Center in Starke, FL. This full-time daytime role focuses on evaluating and developing individualized treatment plans to improve patients' functional independence.

You will document progress, adjust plans based on outcomes, and educate patients and families on adaptive equipment and home safety. A state license and NBCOT certification are required, with 1 year of OT experience preferred.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
