Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join our team at St. Francis Health Services in Milwaukee, WI.

The role supports patients' functional independence through individualized treatment plans and collaboration with families. Requirements include a Master’s or Doctorate in Occupational Therapy, a current WI state license and NBCOT certification, and at least 1 year of experience preferred.

Qualifications

• Master’s or Doctorate in Occupational Therapy required.
• Current, unencumbered WI OT license; NBCOT certification required.
• 1 year of previous experience as an OT preferred.
• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities
