Continuum Therapy Partners is seeking an Occupational Therapist for a part-time daytime schedule at Guardian Angels Health & Rehabilitation Center in Hibbing, MN. Requires a master's or doctorate in OT, a current state OT license, and NBCOT certification; prior OT experience is preferred.

• 1 year of previous experience as an Occupational Therapist preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
