Job summary

Continuum Therapy Partners, LLC is seeking an Occupational Therapist to join our Mount Vernon, IL team. The role focuses on patient-centered care in a multi-site network committed to quality outcomes.

Qualified candidates hold a Master’s or Doctorate in Occupational Therapy, possess an active state license and NBCOT certification, and have a preference for at least one year of experience. This is a part-time daytime position supporting our mission to enhance patients’ independence.

Qualifications

• Requires Master’s or Doctorate in Occupational Therapy.
• Must hold an in-state valid OT license and NBCOT certification.
• Typically 1 year of OT experience preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities

• Master's or Doctorate Degree in Occupational Therapy required
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required
• 1 year of previous experience as an Occupational Therapist preferred
• Evaluates and develops individualized treatment plans to improve patients’ functional independence
• Documents patient progress and modifies treatment plans based on clinical outcomes
• Educates patients and families on adaptive equipment and home safety recommendations
