Job summary

Victorian Post Acute in San Francisco is seeking a PRN Occupational Therapist (OTR) to join our in-house therapy program. You will evaluate residents, develop and implement individualized treatment plans, and collaborate with a multidisciplinary team to support post-acute rehabilitation goals.

Qualifications

• Current California OTR license in good standing.
• Strong communication, organizational, and interdisciplinary collaboration skills.

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled occupational therapy services in accordance with the established plan of care.
• Supervise Certified Occupational Therapy Assistants (COTAs) in compliance with California practice guidelines.
• Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and families to achieve optimal outcomes.
• Participate in care conferences, rehabilitation meetings, and discharge planning.
• Complete accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
• Educate residents, families, and caregivers on adaptive equipment, home programs, safety strategies, and functional independence techniques.
• Coordinate appropriate durable medical equipment (DME) to enhance resident safety and independence.
• Maintain a safe, organized therapy environment and promptly report equipment concerns.
